Document Release Form
Expedited Certification Program- Use to contract expedited inspection services for highly time-sensitive new certifications, add acreages, new facilities, and new equipment.
Rush Review Request- Use this form to request a rushed review of an Organic System Plan (OSP) update, such as a label, input, product, supplier, material, or response to a CCOF request. This form may also be used as a cover sheet for any OSP update, regardless of the timeline for review.

Material Review Request (pdf)- For CCOF clients to request CCOF review of materials (fertilizers/pesticides) that do not appear on the OMRI or WSDA allowed material lists. For more information and help, download CCOF's tutorial: How Do I Find Out if an Input is Allowed?

 

Add Acreage Forms and Information- Use to add parcels or acreage to existing CCOF certified organic operations. -Expedited Service Form

County Pesticide Use Report (PUR) Verification Request Form

Parcel Transfer Form- Use to transfer parcels between CCOF certified operations. Rush review request: PDF | E-form
Parcel Withdrawal Form - Used to withdraw/remove parcels from CCOF certified operations. Rush review request: PDF | E-form

Storage Facility Affidavit- Use to verify organic handling practices at storage facilities that are not required to be certified or inspected under the NOP. Examples include coolers, warehouses and distribution centers.

Livestock Specific Forms  
CCOF Feed Ration DMI Worksheet- Use this worksheet throughout the grazing season to document the Dry Matter Intake (DMI) in each ration fed to each class of animal.  Complete a new worksheet anytime the ration for a class of animals changes.
 
CCOF DMI Grazing Season Average Worksheet- Use this worksheet at the end of the grazing season to average the DMI from each feed ration to find the overall average DMI from pasture for the entire grazing season for each class of animals.  
CCOF Table of Common Feed Type Dry Matter %- Use this document to as a reference for average dry matter percentages for commonly feeds.  
Ruminant Slaughter Delivery Verification- Use this form to verify appropriate organic status of animals delivered for slaughter. E-form

Add Additional Organic Product (OSP H2.0-Product Profile form)- Use to add a processed product to your list of certified products. Rush review request: PDF | E-form
Add Service to Existing Operation (OSP 2.4)- Use to add a service to an existing processing/handling operations. Examples include fee for service nut hulling, processing or packaging. Rush review request: PDF | E-form
Add Facility to Existing Certification- Used by certified operations to add a new location, processing facility, or in case of a move. -Expedited Service Form

Add Product Handled Off-Site (OSP 2.5)- Used by CCOF certified brokers, traders, or certified private labelers to add new products to their certification. Applicable where products are not re-packaged or processed while in the CCOF operator's possession. Rush review request: PDF | E-form E-form
Private Label Co-Packer Profile (OSP-H2.6)- For CCOF co-packers to identify private label products and brands they produce. Identifies if the Name and Seal Use Agreement (below) is required. Rush review request: PDF | E-form E-form
Nonorganic Processing Material Affidavit- Used by CCOF clients and manufacturers to document GMO, irradiation, and sewage sludge compliance for nonorganic materials (ingredients, processing aids, additives, etc.)
Nonorganic Material List- Used to list and track all nonorganic materials (processing aids, additives, etc.) that may directly contact organic products during production at your facility.
Storage Facility Affidavit- Used to verify organic handling practices at storage facilities that are not required to be certified or inspected under the NOP. Examples include coolers, warehouses and distribution centers. Rush review request: PDF | E-form

Organic Percentage Calculation Worksheet- Used by CCOF clients to assist them when calculating the organic percentage of finished products when ingredients may not contain 100% organic content.

Winery Profile (OSP V2.0) - Describes your winemaking process and all potential ingredients and processing aids that may be used.  It must be submitted before winemaking begins and updated before new materials are used. Rush review request: PDF | E-form

Wine & Label Approval (OSP V2.1) - Submitted when wines are finished, to facilitate label review, or to add specific wines to your Client Profile for use in the TTB label approval process. Rush review request: PDF | E-form

Export and Transaction Certificate Request Cover Sheet- Required cover sheet and guidance document for all export and transaction certificate requests. Be sure to include the appropriate request form(s) below. All request forms can be downloaded below.

Canada Extended Affidavit- Used for verifying US/Canadian Equivalence for raw produce, ingredients, or processed products there were produced by companies other than your own. This document is not required if your supplier's certifier has provided alternative verification of US/Canadian Equivalence through a client profile or other attestation document.
For CCOF Certified Suppliers: CCOF provides COR equivalence verification on client profiles through enrollment in our Global Market Access program.

EU Export Certificate Request Form (Vessel)– For shipments to the European Union by sea.
